1) APPLICATION
Applications are submitted via e-mail to acutab@acibadem.edu.tr after the relevant REQUEST FORM for the requested test is completed and signed.
Once the request is evaluated and approved, the full service fee is deposited into the specified bank account. The payment receipt is forwarded to acutab@acibadem.edu.tr The samples to be analyzed must be delivered to ACUTAB in the quantity and packaging specified by our Unit, affixed with a label containing descriptive information about the sample (details requested to be included in the report).
2) PRICING
The fees in the Test/Analysis List do not include 20% VAT. ACUTAB reserves the right to change the fees and/or the pricing system without prior notice. Provided that the service fee is transferred to the ACUTAB account, no fee increases will be applied to the test/analysis services specified in the work request form during the project period. If there is a change in the type of test/analysis service requested during the project period, the current fee schedule will be applied.
3) REPORTING
Only a single report is issued for the tests/analyses requested in the application form. If separate reports are requested for multiple samples, this must be specified in the request form. If a report in a second language is requested, it will be subject to an additional fee. The analysis report and the invoice will be sent to the address specified in the request form. Note: If the return of the samples via courier is requested after the analysis, the shipping costs shall be borne by the institution/individual requesting the test/analysis, and the shipment will be sent on a receiver-pays basis.
